What is Azithromycin?
Azithromycin is an antibiotic used to treat several bacterial infections, but most commonly for STIs like chlamydia. While Doxycycline is often the first-line treatment for bacterial infections, Azithromycin remains a widely used and effective alternative. It is also more suitable for pregnant women or those needing a shorter course of treatment.
At 121doc, we prescribe Azithromycin tablets to treat chlamydia, with a single dose of two 500 mg tablets usually enough to cure the common STI.
How does Azithromycin work?
Azithromycin works by preventing bacteria from making a protein they need in order to reproduce. By stopping the bacteria from spreading, Azithromycin gives your immune system time to kill the remaining bacterial cells, clearing the infection from your body.
Unlike other antibiotics that require lengthy treatment courses, Azithromycin can clear infections like chlamydia in a single dose. Simply take the amount prescribed, and let your immune system do the rest.
What is Azithromycin used for?
Azithromycin can treat several bacterial infections, not just chlamydia. The most common ones include:
- strep throat
- bronchitis
- pneumonia

How effective is it?
According to clinical trials, Azithromycin is up to 93.5% effective as a treatment for chlamydia, depending on the site of the infection.
Here are the results of one clinical study in more detail:
- Genital chlamydia: Azithromycin was 93.5% effective.
- Anal chlamydia: Azithromycin was 78.5% effective.*
*In this trial, Doxycycline cleared anal chlamydia in 95.4% of patients, making it more effective than Azithromycin for this type of infection. For infections in the anus, Doxycycline may therefore be the preferred treatment.
How long does Azithromycin take to work?
You should feel better within a few days of taking Azithromycin for chlamydia. If you have symptoms, you may still feel them for up to a week. This does not mean the treatment isn’t working.
Despite likely feeling better after a few days, you can still be contagious for up to 7 days after your dose. Avoid sex during this time so you don’t risk passing on the infection to somebody else, or reinfecting yourself before the infection has been fully cleared from your system.
To check whether treatment was effective, you should retest 3-4 weeks after treatment. Retesting before this point can cause a false-positive result, as leftover bacterial DNA can remain in the genital tract for a couple of weeks.
Who can use Azithromycin?
To be prescribed Azithromycin from 121doc, you must be:
- over 18 years old
- diagnosed with chlamydia
The first-line treatment for chlamydia is Doxycycline, so Azithromycin is usually only recommended when Doxycycline is unsuitable. This may occur in cases of:
- Pregnancy: Doxycycline may pose a risk to your developing baby. Please note that a doctor may still deem Azithromycin unsafe during pregnancy and prescribe alternative treatment.
- Allergy or intolerance to Doxycycline: Azithromycin contains a different active ingredient, so may be safe where Doxycycline is not. Tell your doctor if you believe you may be allergic.
Can you drink alcohol on Azithromycin?
While it’s technically considered safe to drink alcohol on Azithromycin, it is generally advised to avoid alcohol—or, at least, limit your intake—during treatment. This is because the combination of antibiotics and alcohol can worsen side effects or reduce the effectiveness of the medicine.
With that being said, the interaction is less severe with Azithromycin than it is with other antibiotics like Metronidazole. Consult your doctor for personalised advice.

Azithromycin can produce side effects, although not everyone will get them. The most common side effects of Azithromycin include:
